Rental Application Instructions

Below is an application to rent an apartment from HomeStretch or to apply to be added to the Rental Waiting List if there are no vacancies. Simply push the “print”: button on your browser’s toolbar to print the application and these instructions. 

You will need to fill out the following forms:

  1. Rental Application
  2. MSHDA Authorization for the Release of Information
  3. MSHDA Family Composition
  4. MSHDA Checklist (five pages)
  5. MSHDA Verification of Resources
  6. MSHDA Verification of Earnings

Once you have printed the application, fill out all the information. Follow the instructions below to fill out the forms:

  1. All persons who will be living in the home must be listed.  If you currently live with others who will not be moving with you, do not list them. All adults (18 or older) must sign the application.  All signatures must be dated on the same day or on date later than the Checklist.
  2. All adults in the household must sign the MSHDA Authorization for Release of Information and Privacy Act Notice.  They must also enter their Social Security number on the form and enter the date the their signature. All dates must be the same or later than the date on the Checklist.
  3. Each adult in the household must complete a MSHDA Checklist. This is a critical document and all other forms and verifications must be dated on the same day or later than the date on the Checklist. If ther are any “Yes” answers on the Checklist, provide verification, such as bank statements, proof of pension, proof of Social Security, support, pay stubs, etc.
  4. Fill out only the top (Section A) of the MSHDA Verification of Resources form and sign on the appropriate line.  We need a separate form for each bank or financial institution where a household member has an account. You must provide a contact and a fax number to which HomeStretch can fax the form(s) as it is mandated that the form(s) go directly from HomeStretch to your financial institution(s) and return directly to HomeStretch from them.  You may make copies of the form if needed or print extras.
  5. Fill out only the top (Section A) of the MSHDA Verification of Earnings form and sign on the appropriate line.  You will need a separate form for each employed person. If you or another dault in the home hold more than one job, you will need a form for each employer. You must provide a contat and a fax number where each form is to be sent.  We must send the form from HomeStretch directly to your employer and the form must return from the employer directly to HomeStretch. You may wish to alert your employer that the form will be coming and ask them to complete and return it promptly to HomeStretch. You may make extra copies of the form if needed.

All adults who will live in the household must sign the application. Bring, mail or fax the application and the requested verifications to HomeStretch.  Our address is:

You may wish to access the forms directly from the MSHDA site. Below are links to the forms.  The files on the MSHDA site are in Word format and you will need that program, or a program that can read the Word format,  to view and print the forms. These links will take you off the HomeStretch website to MSHDA site.
